How to submit proof of vaccination or apply for a medical or religious exemption

 

Current Employees

Current employees must upload their vaccination card or exemption form into the Medicat® Patient Portal. Access the Medicat Patient Portal by logging on to your MyRIC account.

If you are having issues logging into Medicat, please email a copy of your COVID vaccine card to healthservices@ngskmc-eis.net

New Employees

All new employees must provide one of the following: proof of vaccination  (1st and 2nd dose or J&J), completed medical exemption application or completed religious exemption application. (NOTE: any individual that has completed at least one dose may not claim a religious exemption)

Where to send documents:

Proof of vaccination, medical or religious exemption applications must be emailed to healthservices@ngskmc-eis.net. The Center for Health and Wellness will review the documentation and follow-up with HR.

Follow-up Notification to Employee:

HR will notify the employee when the Center for Health and Wellness has completed the review process and HR will confirm the employee’s employment status.

Report a faculty and staff case

Faculty and staff who have tested positive for COVID-19 or are experiencing symptoms, or who may have had a significant contact with someone that has tested positive for COVID-19 should contact the RIC Office of Human Resources at 401-456-8216 or complete the COVID-19 Faculty/Staff Report Form. This information is shared directly with Human Resources for appropriate follow-up. If you are experiencing symptoms, receive a positive COVID-19 test result, or may have been exposed to someone with COVID-19, please remain off campus until you are cleared by Human Resources to return to campus. Employees with questions should contact the RIC Office of Human Resources at 401-456-8216.

In difficult times like these, we are especially thankful to the staff who come to campus each day to ensure that the college’s critical operations are maintained. As the campus adjusts to a new environment where our students learn remote and faculty members teach remote, teleworking arrangements provide an opportunity for some staff to perform the essential functions of their job from an alternate work location.

If appropriate, you should discuss with your supervisor your eligibility to take advantage of a teleworking arrangement.

REMINDER - REGARDING YOUR EMPLOYEE ASSISTANCE PROGRAM (EAP) BENEFITS:

The State’s EAP is a completely free benefit that’s available to employees and their immediate family members. It provides information, referrals, and support to help you reach personal goals and bounce back from hardships, including the latest COVID-19 support resources

  • Effective June 1, 2021, administration of the State’s EAP transitioned from Optum to Blue Cross & Blue Shield of Rhode Island (BCBSRI). The BCBSRI EAP is administered in conjunction with Beacon Health Options (Beacon), using their “Beacon Wellbeing” platform.
  • Access the State’s EAP anytime (24 hours a day, 7 days a week) by phone or online:

A. Phone: Call the BCBSRI CARE Team at 866-987-3705 or 401-429-2104, and select option 2 to reach a Beacon Employee Workplace Counsellor.
